Hi Jane, It is great to hear from you. I sure wish I could help you out with info on the planner, but it is a picture that came with the Homeschool Mother’s Journal link-up and isn’t a planner that I actually use here in my home. A place like RainbowResource.com or somewhere like that would probably list out all of their homeschool planners and you might find something similar there.
